Entry Forms for Participants
For every event that an athlete participates in, an entry form must be filled out and sent to the office by the due date. We cannot stress this point enough. We need specific information for each event and that information is on each entry form. If we do not receive an entry form, your athlete is not eligible to participate in the event. Please do not disappoint your athletes by not sending in your entry form.
